Dr. Brandon Sinn, DVM has done OFA clinics all over the Midwest. His goals are to offer a low stress environment and attain radiographs without the use of sedation. Late fees for submissions after Oct 16 including day of clinic.

There are 3 steps for the OFA process.

STEP 1: Fill out separate OFA forms for each test for each dog you have that you are registering that Dr. Sinn will submit, along with a method of payment for the OFA fees (usually a check or credit card).  Bring these forms to the show clinic. The link below takes you to the page at the OFA website.  Follow their instructions to either fill in online or download and print a form to fill out manually. The forms:  https://ofa.org/applications/ Step2: Fill out TSKC's reservation form for examinations (one for each dog). Step 3: Complete payment for each dog. Note:  You will need to do steps 1 - 3 for each dog you have. Substituting a dog after the end date of Oct 16 will incur a late fee.  TSKC is collecting fees for the examinations now, and the OFA costs are in addition to the exam fees and are paid at the time of the examination. Please check in at TSKC's OFA table (Southeast corner of hall with rings) on Saturday, November 1, no later than noon so we can ensure that everyone can be accommodated promptly. More Details:

Large Dog Fee is needed only if you have 1 or more X-rays in your choices, and only once per dog. The fee depends on the weight of your dog. Note that other tests such as Basic Cardiac, Dentition, and Patellar Luxation do not require lifting onto the table and do not need to pay the extra fee. CD/Email/Flash drive Files: If you wish to have copies of the X-rays, choose this option. If you wish to have them copied to your flash drive, remember to bring it with you, as Dr. Sinn does not supply flash drives. Note that Basic Cardiac, Dentition, and Patellar Luxation are physical examinations (not x-rays) and thus will not appear on the CD. If you have multiple dogs with x-rays, you only need to pay this fee once and it covers all your files for dogs you own. Additional Info for STEP 3 - Adding the tests to the cart for payment: The same list is repeated: This is for you to add the test(s) you want to Pay for, for the dog in the information in the Registration Form.  You must check all the same boxes again in order to pay for the tests. Second dog: If you have a second dog needing tests, please complete payment for the first dog, then begin the process again from the top for subsequent dogs. You may be able to auto-fill the information the second time. Also when you submit the first form, you should receive an email copy of it as long as your email address is typed correctly.
