Late fees for submissions after Oct 16 including day of clinic.

There are 3 steps for the OFA process.

STEP 1. Fill out TSKC’s form  for registrations for examinations for ONE DOG.

Step 2: Complete the payment for one dog.

Step 3: Fill out OFA forms that Dr. Sinn will submit, along with a method of payment for the OFA fees. (usually credit card or checks)

If you have additional dogs to test, go back to Step 1 and then begin again. Substituting a dog after the end date of Oct 16 will incur a late fee. 

TSKC is collecting fees  for the examinations now, and the OFA costs are in addition to the exam fees, and are paid at the time of the examination.

Please check in at TSKC’s OFA table (South East corner of hall with rings) on Saturday, November 1, no later than noon so we can ensure that everyone can be accommodated promptly. Call Joan at 417-434-7687 if you are unable to check in by noon.

More Details:

Large Dog Fee is needed only if you have 1 or more X-rays in your choices, and only once per dog. The fee depends on the weight of your dog. Note that other tests such as Basic Cardiac, Dentition, and Patellar Luxation do not require lifting onto the table and do not need to pay the extra fee.

CD/Email/Flash drive Files: If you wish to have copies of the X-rays, choose this option. If you wish to have them copied to your flash drive, remember to bring it with you, as Dr. Sinn does not supply flash drives. Note that Basic Cardiac, Dentition, and Patellar Luxation are physical examinations (not x-rays) and thus will not appear on the CD. If you have multiple dogs with x-rays, you only need to pay this fee once and it covers all your files for dogs you own.

STEP 2: Adding the tests to the cart for payment:

The same list is repeated: This is for you to add the test(s) you want to Pay for, for the dog in the information in the Registration Form.  You must check all the same boxes again in order to pay for the tests.

Second dog: If you have a second dog needing tests, please complete payment for the first dog, then begin the process again from the top for subsequent dogs. You may be able to auto-fill the information the second time. Also when you submit the first form, you should receive an email copy of it as long as your email address is typed correctly.

Put a CHECK (or select) the tests and options you want for OFA, and then click “ADD TO CART” button. Please only buy OFA tests on this transaction. FINISH by locating the shopping cart icon in the menu to finish the payment process. If you don’t get an emailed receipt, you haven’t completed the payment process!

Begin by choosing the yellow PayPal Button.

You may pay without a PayPal account. Just follow the PayPal buttons, but instead of logging in, choose to use a different card. You will enter your card number etc. and proceed to pay.

    Click this "SEND" Button for Registration. You should see a green box below this text, with a message confirming you have sent the form. You should also receive an email with your submission, in case you forgot which tests you ordered. You will need those for the second half (payment) of the form!


    STEP 2: PURCHASE TESTS: Below is a second list of items, the same items as in the list above. Put a check in the box for the same items you checked and registered for above. This time, it adds them to the shopping cart for this dog. Then complete your payment via PayPal or another credit or debit card.
